To store: store open and unopened containers in a dry area at room temperature. avoid any extreme temperatures. close the opened container tightly with plastic lid. use contents within 1 month. use by: see above. directions for preparation and use: before using an infant formula, ask the advice of your healthcare professional. powdered infant formulas are not sterile and should not be fed to premature infants or infants who may have immune problems unless directed and supervised by your baby's doctor. your baby's health depends on carefully following these preparation, use and storage instructions. failure to follow these instructions could result in severe harm. before preparing infant formula, make sure to always wash your hands. thoroughly wash bottle and feeding utensils in hot, soapy water before each use. ask your baby's doctor about the need to boil or sterilize water for formula and the proper preparation of bottle and feeding utensils. 1. pour desired amount of water into clean bottle. mixing with water warmer than 104 degrees f (40 degrees c) or warming formula above 104 degrees f (40 degrees c) may lessen the benefits of the probiotic cultures. 2. add the exact ratio of unpacked, leveled powder to water indicated on the feeding chart. 3. cap bottle and shake well until powder is dissolved. feed immediately and discard used formula within 1 hour. note: freshly prepared formula is best. only if necessary, refrigerate freshly prepared, unused formula within two hours between 35-40 degrees f (2-4 degrees c) and use within 24 hours. do not freeze. warm infant formula to room temperature and shake well before feeding. any left over after feeding should be discarded and not stored. use this feeding chart (only use scoop enclosed to measure): to make bottle: 2 fl oz; 2 fl oz water; 2 scoops (8.8 g) powder unpacked level scoop. to make bottle: 4 fl oz; 4 fl oz water; 4 scoops (17.6 g) powder unpacked level scoop. to make bottle: 6 fl oz; 6 fl oz water; 6 scoops (26.4 g) powder unpacked level scoop. to make bottle: 8 fl oz; 8 fl oz water; 8 scoops (35.2 g) powder unpacked level scoop. note: mixing instructions require 1 scoop per 1 fl oz water. each scoop adds approximately 0.1 fl oz to the amount of prepared formula.
